Want a faster and easier way of filling out your job search efforts on California's EDD Continued Claim Form (form DE 4581...). If so, then obtain the word template below and follow this procedure:
Note: This is not California's DE 4581, we cannot provide that, you should have received DE 4581 in the mail. This is a word document that has columns set at the correct spacing to print onto form DE 4581.
1) Type in the date, company, address, person contacted, position sought and the result of each job inquiry in the corresponding column. Or copy the data from a spreadsheet directly onto the table
2) Tear off the perforated section of the continued claim form you received in the mail
3) Load your printer with the portion of the form that must be returned to EDD. Make sure the form is loaded correctly so that your job search record prints on the correct side of the form.
5) Print it!
Vio la ! Now mail your form, and get back to networking and landing that dream job! Good luck :)
